In the patient portal, please wait for an e-mail confirmation before assuming your appointment is set. Also remember to bring an original copy of your insurance information, and your co-pay.  Check in at the front desk in advance in order to fill out your paperwork and verify your insurance. 

Your First Visit:

To save time, consider filling out your registration forms and confirming your insurance coverage prior to the visit.  You can fill them out in the Patient Portal and print them out.  We can also fax the forms to you, or you pick them up at the reception desk prior to your appointment. In addition, you have the option of printing them out below and filling them out before your arrival:
